Why keep a log?

Your doctor sees one reading. Your log shows the weeks in between.

Same times, same arm, two readings, written down exactly.

A home log is only useful if it is easy to keep and easy to read. This one is in large print, with one place for everything: your readings, your medicines, and the questions you want to ask.

Fill it in on screen, or print it and use a pen. Bring it to your appointments.

Blood pressure categories, inside the log

The log includes the American Heart Association categories for adults, what to do if a reading is very high, and the steps for an accurate reading at home. Your doctor may set a personal target for you; write it on My Details and go by it.

Category Top number (systolic) Bottom number (diastolic)
Normal Less than 120 and Less than 80
Elevated 120 – 129 and Less than 80
High blood pressure, Stage 1 130 – 139 or 80 – 89
High blood pressure, Stage 2 140 or higher or 90 or higher
Severe hypertension Higher than 180 and/or Higher than 120
Higher than 180/120? Wait one minute and measure again. If it is still that high and you have chest pain, shortness of breath, back pain, numbness, weakness, a change in vision or difficulty speaking, call 911. Without symptoms, call your healthcare professional. (American Heart Association guidance.)

Questions

Good to know

Can I fill it in on my phone?

Yes, with the free Adobe Acrobat Reader app (or the Files app on iPhone and iPad). On a computer, open it in Chrome, Edge or Adobe Acrobat Reader. Save after each entry. Or print it and write by hand.

Is it dated?

No. Each month has a space for the month and year, so you can start any time.

Is there a free version?

Yes. ElderlyDaily has a free one-month printable blood pressure log. This tracker adds a full 12 months, the daily medicine charts, monthly summaries and doctor-visit pages in one file.

What is a normal blood pressure for seniors?

The American Heart Association uses the same categories for all adults: normal is a top number under 120 and a bottom number under 80. Your doctor may set a personal target for you based on your health, so write that target in the log and go by it.

Does it tell me what to do about my readings?

No. It is a log for recording and sharing information with your doctor. Never start, stop or change a medicine because of your readings without talking to your doctor first. In an emergency, call 911.

Can I print more pages?

Yes. Keep a clean copy ("Save As" before you start typing) and print any pages you need, for your own personal use.
