Dementia care runs on details. Too many of them live only in one person's memory.
Take This to the Appointment — fill it the night before, and walk in with organized observations and your top questions.
The ABC Behavior Log and Sundowning & Evening Patterns pages help patterns become visible — and record what actually helps.
The Caregiver Handoff Sheet and My Loved One at a Glance brief anyone stepping in — in minutes, not a long phone call.

Honest about what it is
An organizational and documentation aid. It is not medical advice and does not diagnose, treat or replace professional healthcare guidance.
The behavior logs are for pattern-spotting, not diagnosis — they help you have a clearer conversation with the care team.
Aging Parent Medical & Emergency Binder
Insurance, documents locator, care team and wallet cards — the wider family binder, designed privacy-first.
Good to know
Where do I start?
With the four foundation pages: the Quick-Start Profile, My Loved One at a Glance, All About Me, and the Master Medication List. They feed almost every other page. You don't have to fill everything — start with the pages that solve a problem you have this week.
How do I fill it in on screen?
Open it in Adobe Acrobat Reader (free), click a field and type, then save. Or print any page and write by hand. The daily pages are made to be printed fresh as often as you need.
Which paper size do I get?
Both — a US Letter version and an A4 version are included, plus a short quick-start guide.
Can the whole family use it?
Yes — it's built to be shared across family members and shift caregivers, so everyone works from the same information.
Is this medical advice?
No. It's an organizational tool. Always follow the guidance of your loved one's healthcare team, and in an emergency call your local emergency number.
Will I get updates?
The last page links to a free resources page with replacement pages and updated versions as they're added — no account needed.
